No charge: dental exam, x-rays (unlimited), two cleanings (prophylaxis) one every 6 months per membership year, fluoride treatment for children, unlimited emergency consultations.

• Procedures not listed or performed by an in-house specialist will be given a 10% discount off the usual and customary fee.
• Plan based on anniversary day yearly renewal.
• Benefits not used will expire on anniversary date.
• Benefits are not transferrable.
• This plan is not combinable with dental insurance.
• Prices subject to change upon renewal date.
• Membership fee will not be refunded after first visit of at least one member on the plan.
• Gorbatov Dentistry broken appointment/short notice cancellation policy applies.
• It is the patient’s responsibility to schedule their second cleaning no later than 45 days prior to plan’s expiration date.
• Private Dental Plan is not a dental insurance policy.
